On 09/09/2010 a note was written by a social worker on the wrong patient. The patient whose record this note was located in requested a large date range of their records for a regional office claim and records including the note on the wrong patient was released on 03/27/13. The patient gave the records to the Regional Office on 04/19/2013 and the Regional Office contacted the Omaha Medical Center Privacy Officer on 04/22/13 to report they found a note entered on the wrong patient. The note was reassigned to the correct patient and is no longer in the incorrect chart. (English)
