Veteran B notified the Privacy Officer of a travel claim form that was sent to him in error. It was determined that when the VA Travel Office was attempting to mail a claim for reimbursement of travel expenses to Veteran A, they inadvertently mailed that form along with a Travel Letter to Veteran B. Information on the travel claim form belonging to Veteran A included his full name, last four of his SSN, and home address. (English)
