A research assistant never came back to work as a result of having mental and relationship problems. It was found later that there were files missing from a research study between the VA and the UNMC hospital. This research assistant last had these files. The files contained one VA participants name and social security number on an informed consent for the study and a questionnaire about 7332 information relating to the participant. (English)
