Minneapolis Housekeeping staff contacted the ISO to report that 3 bags of trash containing sensitive information had been found in two areas of the facility.Incident UpdateThe contents of the bags were:Bag #1 found in room 2V114 contained the following:2 prescription authorizations for eyeglasses containing name, full SSN, DOB, eyeglass prescription and the Date of service1 Confidential Medical worksheet summary containing name, last 4 of the SSN and Date of service Bags #2 and #3 found in room 3V182 contained the following:5 labels containing name, full SSN, DOB, and allergies.Therefore 7 Patients will recieve a letter offering credit protection services and 1 Patient will receive a notification letter.
